Fill out the Online Partner request form.
FAX a copy of your Air Carrier Certificate and Current Insurance to 888-313-5387.
Please have your Insurance carrier add Ameristar Jet Charter, Inc.
to the list of Certificate holders on your Insurance policy. Please
give the following information to your Insurance Carrier:
